Respite Care

When the family are the main caregivers for a participant, they may need to take a break for their own well-being. Whether it be for a casual period, or a short period on a regular basis, Upliving will be able to provide personal care and assistance (community access and in-home services) for a participant.

Therapy Support – Physiotherapy Program Implementation

We can help participants in completing an existing physiotherapy exercise program at home, that has been developed and prescribed by a qualified physiotherapist.

Support and Plan Management

Level 1 and 2 Support Coordination

We can help participants understand, implement, and manage their NDIS plans to increase their ability to navigate the NDIS, connect with appropriate services, and coordinate supports effectively to achieve their goals, where their needs are not of a complex nature requiring specialist intervention. Support Coordination involves establishing service agreements, monitoring budgets, resolving issues with providers, preparing for plan reviews and transitions, reporting to the NDIA, and promoting participant safety and well-being.

Plan Management

We provide financial intermediary support to assist participants in managing the financial and administrative aspects of their NDIS plan. We can handle provider payments, tracks budgets and provide financial reporting, allowing participants to focus on achieving their goals without the burden of paperwork and compliance

Accommodation

Supported Independent Living (SIL)

With the support of SIL Accommodation, participants are able to live freely, whilst our support workers assist with daily tasks such as preparing nutritious meals, maintaining house cleanliness, supporting with personal care and activities, providing transport and access to community activities and appointments and assisting with managing medication. Participants will be supported in finding accommodation, whether on their own or in shared accommodation depending on their NDIS funding. 

Short-Term Disability Accommodation (STA)

Upliving provides temporary care and accommodation for participants away from their usual home. It is often referred to as respite care, but STA encompasses more than just giving carers a break—it also supports participants in building independence, social skills, and accessing new experiences.

Specialist Disability Accommodation (SDA)

SDA is a range of housing designed for people with extreme functional impairment or very high support needs. We can assist with coordinating suitable, compliant SDA housing.
